Register your intention to receive a vaccine at UHN

The University Health Network (UHN) has launched a vaccine registry for patients with high risk conditions. This is not a system for booking appointment. It collects your personal health information so UHN can contact you by email once a COVID-19 vaccine appointment at a UHN sponsored location is available.

To register in this system, you only need to meet 1 criterion in Box 1 AND 1 criterion in Box 2.
Red indicates conditions that would be the most relevant for Ontario patients of our Dalglish Family 22q Clinic:

Box 1
You must meet at least one of the following criteria:
  • First Nations, Métis and Inuit Adults
  • Recipient of Chronic Home Care
  • Faith Leaders
  • Organ transplant recipients
  • Hematopoietic stem cell transplant recipients
  • People with neurological diseases that may compromise respiratory function
  • Haematological malignancy diagnosed less than 1 year
  • Kidney disease eGFR less than 30
  • Obesity (BMI over 40)
  • Other treatments causing immunosuppression
  • Intellectual or developmental disabilities (Note: 22q11.2DS is a developmental condition)
  • Essential caregiver for a member of a priority group

AND

Box 2
You must meet at least one of the following criteria:
  • Be a patient receiving care at University Health Network (UHN) (Note: All of our patients meet this criteria), Sinai Health, Women’s College Hospital, or Toronto Grace Health Centre.
  • Live in our catchment area (south of Eglinton Avenue West to north of the Waterfront, east of Roncesvalles Avenue and west of Yonge Street).
  • Have your primary care provider (family doctor or nurse practitioner) within our catchment area (south of Eglinton Avenue West to north of the Waterfront, east of Roncesvalles Avenue and west of Yonge Street).

If you meet at least 1 criterion in Box 1 AND at least 1 criterion in Box 2, please go to the vaccine registry to register your intention to be vaccinated. If you need help using the registry or booking a vaccination appointment, please call us at (416) 340-5145 or email us at 22q@uhn.ca.
